claimID also does this - and was the first to implement the multiple
OpenIDs per account.

We provide a simple list - as well as inform the user which one is
'active' (the one they used to log in). Of course, the user cannot
'delete' the active one, but has the opportunity to delete/manage any
others in their list.

Simple, clear, and gives the user full control.

The only limitation we have on the multiple OpenIDs per account, is that
a single OpenID cannot be listed in different accounts - so that if
someone tries to 'add' an OpenID to their account, it simply fails. It
is only a one-to-many relationship.
