Login dialog strawman based on IIW discussion

Pat, I've been trying to mock one up with a tab, but I'm not managing  
to produce anything that looks reasonable. For example, I don't where  
to put the "Please log in" label (or equivalent) without  
substantially increasing the size of the whole box.

Do you have an example you could point us to, or could you mock one up?
