Issue Title: Behaviour of Polling Endpoint When Paused or Disabled

Comment: > so there is currently no status or diagnostic issued to receivers when a stream is paused or disabled. This is untrue. Section 8.1.5 says that a Transmitter MUST send a StreamUpdated event if it changes the status of the stream without a request from the Receiver. We should treat polling a paused/disabled stream the same way we treat polling an empty stream - the Transmitter responds with an empty list. The enabled/paused/disabled state effects whether new events can be _added_ to the stream, not whether information can be sent out from the stream. I propose that we close this issue as WON'T FIX
