Issue Title: Incorporate Section 10 earlier in the spec

There has been some confusion from implementers because the details of push/poll are down in section 10. Those details should be moved up and split up. Section 4 becomes info about events (include 10.1, 10.2) A new Section (inserted after the current section 7) becomes Delivery (include 10.3).
