[Openid-specs-risc] Issues based discussions in WG meetings

Hi all,
Instead of writing down everything we discuss in the HackMD notes, I'm
thinking of trying out a method when we are discussing specifications
related issues, we should make GitHub issues, and discuss them in the
bi-weekly meeting. Anyone taking notes should capture any discussion we do
directly as a comment in the GitHub issue.

Of course, there will be other items we discuss that cannot / should not be
GitHub issues, which we will continue to capture in HackMD.

With that in mind, most of the agenda should simply refer to the issues.
I've added Tushar's item to the agenda:

Please identify or create issues that you would like to discuss in the
meeting on 11/19.
