[Openid-specs-risc] Specs rename with a twist

Hi all,
As a part of renaming the current "Shared Signals and Events Framework
Specification" to simply "Shared Signals Framework", I've also migrated the
source from XML to MD (manually and rather painfully). Please review the
pull request, mainly to provide feedback on any unintended changes or
omissions in the updated spec. There are some known issues that I have
noted in the comments of the PR.

I will be updating the other specs (CAEP, RISC) to refer to this spec soon.
