What I want is immaterial. What I want is to improve the outcomes for as many people as soon as possible and expand from there. I think after 10 years of effort we’d stop dreaming about what could be and get serious about what is practical and sufficient for the majority of people (while allowing any one to opt out if that is their preference).
Arguing with the SPM statements is like arguing with the goals of “Voice of America”.
Do I want to live in a world where everyone is free and able to select their leaders and speak their mind? Yes.
Would I settle for fewer people supporting stateless acts of terrorism and diplomatic settlements of disagreements amongst sovereign nations?
Yes.

Aaron and Glen, does what the SPM folks said feel right to you, particularly the points in bold?
* As the patient, I am the primary steward of my health information wherever it is.
* As the primary steward, I have full access to all of my health information.
* As the primary steward, I can determine who has access to which segments of my health information.
* As the primary steward, I am notified about changes and access by others to my health information.
* Stewardship of my health information is done through a single tool (web page or application) across my service providers.

I’m still struggling with a very simple use case and how to express it using HEART.
As a Medicare recipient, I have supplemental insurance and Medicare Part D. In addition, I have a primary care physician, a handful of specialists, a dentist, a periodontist, a preferred drug store, a mail-order pharmacy, a preferred radiological imaging practice, a preferred lab company, an occasional massage therapist, two designated people to make decisions if I am unable, and several online medical records. Privacy is not my main concern; information sharing is key. I want all of those who provide health care and related services to have the same information and share it among themselves freely. Then I will depend less on my memory and record-keeping. It is all a bit tenuous now, and I feel burdened by having to manage it all.
So, how do I relieve myself of the management burden, let every provider and insurer share all the data, get to see my health data in one place, ensure that the two people I’ve designated also have the same access, and avoid having my data used for marketing or financial purposes?
I don’t expect HEART to be the solution, of course, but it needs to be part of it.
