[Openid-specs-fapi] FAPI 1.0 errata edit strategy

Right. I am not a CSS person and that language is hard! I was also thinking
of asking someone for help in this regard.
What I am noticing right now are
1) The toc wrap for the first line is not working.
2) The toc does not collapse.
3) The main body has too much margins for small screen.
Let me know if any other problems.
