[Openid-specs-fapi] Pull requests to resolve open part 1 & 2 issues

Hi all,

As discussed on the this week's call, we want to try to progress some of the open issues via email etc.

With that in mind, I’ve opened pull requests that I believe close off 6 of the open tickets:

It would be greatly appreciated if comments (positive, negative, or ambivalent) could be made on the relevant pull requests in a timely manner please.
