Regarding pagination I also raised (yet) another PR to find a compromise between the two camps.

It mandates support for token-based pagination and introduces offset and limit/size as two separate capabilities. When combined those capabilities also allow page-based pagination when the PDP supports it.

As for issue 375; I believe this was addressed by https://github.com/openid/authzen/pull/364. Or is that still missing something?
