[Openid-specs-ab] OP Commands miscelaneous issues

Hey

I implemented an RP endpoint and OP test framework to exercise the
protocol and found a few things to fix. I also had a call with @rexxars who
has worked with SSE since it started and maintains some of the popular NPM
packages for EventSource to get his learnings.

I'm planning on putting all these small changes into one PR unless any of
them become contentious.

I'll send separate emails for proposals that I expect are more likely to
have discussion.
