Change the description of OP-B-08 from "Includes c_hash in ID Token when Code Flow is Used" to "ID Token has c_hash when ID Token and Authorization Code returned from Authorization Endpoint".

While you're at this, please verify that this test is shown for the "code+id_token" and "code+id_token+token" flows.
