The definition sometimes includes "A" and sometimes not. e.g., 

    Authentication Context Class Reference
    Identifier that identifies the Authentication Context.

    Claim
    A piece of information about an Entity that a Claims Provider asserts about that Entity.

ISO Directives say that you MUST NOT include the articles nor trailing period (".") so that the defined term can be replaced by the definition. I think it is a good practice, but even if we do not follow it, we need to be consistent.
