[Openid-specs-ab] Issues on hold

We have 39 issues on hold:  http://hg.openid.net/connect/issues?status=on%20hold.

Some of these are on hold until we go final but some of them we deferred to consider until after we reached Implementer's Draft status.  We should go through these sometime soon and decide which we should take off hold now and which we can resolve.
