[Openid-specs-ab] Session Management Support

My OP supports Session Management.

There are some points that are not clear.

1. Origin URL

The past Dynamic Registration specs included js_origin_uri, but there
is not it now.
How does OP generate origin URL from client_id?

2. Discovery spec

My OP does not yet include two endpoints of this spec to openid
configuration response.
It is necessary for Discovery spec to be updated.

- add OP iframe URL as a "check_session_endpoint"
- add OP Logout endpoint URL as a "end_session_endpoint" or "logout_url"
- remove "refresh_session_endpoint"
