[Openid-specs-ab] Issue Tracking How-to page

Since bunch of you are going to be producing issue tickets etc., I have
created a page that explains how it should be done.

I wanted to make it the hg.openid.net but I have not figured out how to do
it yet.
Anyways, for the time being, it should work.
