[Openid-specs-ab] Core 1.0d03

Since I could not sleep due to jet lag, I took liberty of combining the Core
and Session Management as it has been planned.
While doing that, I found out that openid claim and id_token claim are
complete overlap for the REQUIRED claims, with the exception of server_id
which was present only in openid. The openid claim has bunch of other OpenID
2.0 compatibility claims but they are all OPTIONAL.

Thus, I have combined the two claims and used the parameter name "openid".
Hope that is ok.
It should also be good for marketing reasons.
