[Marketing] OpenID collateral draft 1 -- feedback please

We need a standard set of collateral that we can hand out at events,  
and that nobody is going to get upset about ;-)

Attached a very basic first draft. (Thanks, Bill, for starting this)
  - membership application form and terms and conditions taken  
straight off the web
  - the February press release
I took those and left out everything else because I need to print them  
prior to leaving for EIC, and I didn't want to start a discussion on  
how to position OpenID and the foundation as part of this process.  
Certainly we need more stuff, including 1-page backgrounder, benefits  
of joining, chapter orgs etc. but we don't have those for now.

If you have any feedback, please provide it to me by the end of the  
day today -- yes, this is a last-minute job, and I'd appreciate very  
specific requests over cans of worms ...
