[OpenID] Decommissioning demand.openid.net

After not hearing back on this thread, we've gone ahead and shut down
demand.openid.net and redirected it to the wiki page:

http://wiki.openid.net/Demand

You can still see the app here:

http://openid-please.appspot.com/

And the source code is on Google Code:

http://code.google.com/p/demand/
