Rarely, but not often enough, IMO.

On the public IETF list, it's no more than a handful.  On working group lists, 
probably less than that.

The general desire to maintain an open environment leads to a reticence at 
terminating disruptive behavior.  Most folks who are in charge of lists do not 
have a solid enforcement model and since they are committed to openness, they 
tend towards inaction.

The critical error is in misunderstanding just how destructive these disruptive 
folk are to the productive work of a list.
