We had account login and OpenID login on same page but different forms.
We also found it useful to allow users to link other OpenIDs to the
same account and to have a separate table for OpenIDs that linked the
OpenID to an account, This allows you to now change existing table as
well as allow multiple OpenIDs to the same account.
