If there is no assoc_handle in the checkid_setup request, the most
likely cause is that an attempt to establish an association failed.
That, combined with the fact that the work-server is never receiving
check_authentication messages, makes Pete's diagnosis quite likely; it
sounds like there is something preventing the RP from making direct
requests to your work-server.
