[OpenID] new example Rails/OpenID app

I've found there's a fair bit of boilerplate code involved in getting an 
OpenID-enabled app going with Ruby on Rails.  So I wrote the simplest 
app I could as an excuse for the openid controller, helpers and filters: 
a Simple Registration guestbook.
