I'm not sure if anyone else is monitoring this but the HIMSS Identity Management Task Force has a special committee working to "develop guidance as to how to designate a proxy or delegate and give the delegate access to the patient's PHI with the same level of confidence".

This group met last week to document a set of proxy use cases that describe the multiple patient-to-proxy relationships that exist in HC. (spouse-spouse, parent-child, adult child-parent/grandparent, etc.)
If this group is interested and if you think it will be helpful to our work at hand, I can try to provide the complete set of use cases documented by the HIMSS TF.
